Tanya Dougherty, PharmD, BCPS, Clinical Pharmacy Specialist, Ambulatory Care, University of Pennsylvania Department of Family Medicine & Community Health, Penn Presbyterian Medical Center
Dr. Tanya Dougherty is currently a clinical pharmacy specialist in ambulatory care with the Department of Family Medicine and Community Health in Philadelphia, PA. In this position, she assists in population medication management within a patient centered medical home practice to create a pharmacy home model of care. Dr. Dougherty serves as one of two appointed pharmacist members on the Pennsylvania Department of Human Services Patient-Centered Medical Home Advisory Council.
Jamie McConaha, PharmD, CGP, TTS, BCACP, CDE, Assistant Professor of Pharmacy Practice, Duquesne University Mylan School of Pharmacy
Dr. Jamie McConaha is an Assistant Professor of Pharmacy Practice in the Department of Clinical, Social and Administrative Sciences in the Duquesne University Mylan School of Pharmacy. Dr. McConaha maintains a clinical practice site with Preferred Primary Care Physicians, an accredited patient-centered medical home and accountable care organization. In this setting, Dr. McConaha offers a variety of clinical services for PPCP patients including medication therapy management appointments, transition of care consults, and tobacco cessation classes. Dr. McConaha is a former member of the PPA Board of Directors, and is currently serving a 2-year term on the Patient Centered Medical Home Council for Pennsylvania Medical Assistance.

Program Objectives: At the completion of this activity, the participant will be able to:
1. Describe the current legislative status of Medicaid CMM reimbursement in Pennsylvania.
2. Identify areas of opportunity for pharmacists in Pennsylvania to participate in Medicaid payment systems around medical homes.
